We also offer care coordination support through our licensed social worker, helping families connect with community resources, organize services, and navigate care needs. Our goal is to help seniors remain comfortable, safe, and supported in their homes and communities.

Do you have social workers on staff?

Yes. Lean On Me Senior Services works with licensed social workers who assist with care coordination, support services, and guidance for seniors and their families. Social workers can help connect clients with community resources, provide support during life transitions, and assist families in navigating senior care options.

What does care coordination include?

Our care coordination services help families organize and manage the support their loved ones need. This may include coordinating appointments, connecting families with community resources, assisting with post-hospital transitions, and ensuring seniors receive the appropriate support services.

Do your social workers provide medical treatment?

No. Our social workers do not provide medical treatment. Their role is to provide support, guidance, and coordination of services to help seniors and families navigate care options and maintain quality of life.

Do you provide medical care?

No. Lean On Me Senior Services provides non-medical support only. We do not administer medications, perform medical procedures, or provide nursing care. Our services focus on companionship, assistance with daily activities, and supportive care.

Can you accompany seniors to appointments?

Yes. Our companions can escort seniors to medical appointments, social outings, community activities, and errands. We provide transportation assistance and supportive companionship to help seniors feel safe and confident when leaving their homes.

Do you offer post-hospital support?

Yes. We provide post-hospital companionship support to help seniors transition safely back home after a hospital or rehabilitation stay. Our companions can assist with transportation, errands, daily activities, and companionship during recovery.

Do you serve seniors in facilities as well as in their homes?

Yes. We provide services in clients’ homes, assisted living communities, rehabilitation centers, and other care facilities.

How do you ensure safety?

Our team provides supervision, gentle reminders, and regular check-ins while reporting any concerns to designated family contacts.

Lean On Me Senior Services provides non-medical companionship and support, including transportation to errands and appointments, companionship visits, reminders (including medication reminders only), observation, and general well-being check-ins.

For safety and compliance, we do not provide medical care or hands-on mobility assistance, including lifting, transfers, wheelchair assistance, or bed assistance.
